Clear aligners 
Invisible aligners

 

Clear (so-called invisible) plastic aligner digital systems (eg Invisalign, Clear-smile or eCligner) have been marketed for the last ten years. Before that, orthodontists may still have constructed a similar suck-down clear retainer - with the teeth being moved manually on a model in the laboratory.

 

The orthodontist sends a digital record of the patient's teeth to San Francisco or Korea. The company-computer then breaks down the necessary aligning movements into approximately 30 stages. Agreement concerning the end-stage alignment is reached online before the actual aligners are produced and shipped.

 

Patients are then instructed to wear each aligner as close to full-time as possible - for 2 to 3 weeks before moving on to the next one. At each stage, the orthodontist will make minor adjustments (such as opening small spaces to aid in the relief of crowding). The key to success is patient compliance.

While clever clinicians are able to provide some three-dimensional control, it does have to be pointed out that these are not major tooth-moving appliances and they do not provide the same opportunities for major realignment and bite-correction as fixed braces do. They should be chosen therefore only after a full and honest discussion of all treatment options and their respective advantages and disadvantages.

In the end, all roads still lead to long-term gum care and management and long-term, if not life-time, retention. This is true for braces on the inside and outside, as well as for any type of clear plastic aligner treatment.
